This module focuses on care transitions, such as changes in health status, care settings, or caregiver roles. You will learn how to plan and communicate during transitions to reduce stress and confusion. 
This is important because transitions are emotionally challenging and, when poorly managed, can disrupt care and increase risk for both caregivers and care recipients.
